Assessment

Prior to implementation of formal services and training, a comprehensive assessment is conducted to determine the individual’s current skill levels, needs, and interests.

The assessment process allows staff to gather background information, such as age at the onset of each disability, the severity of losses, motivation, and level of coping.

The process includes formal testing, consultation with other involved community providers, and interviews with the client and family members.
Careful analysis of all information by the staff leads to the design of a service plan with realistic goals and outcomes.

Assessment identifies options in areas including, but not limited to:

·        Communication methods

·        Independent living skills

·        Orientation and mobility (safe travel)

·        Employment

·        Leisure activities and interests

·        Technology to optimize remaining hearing and sight
